All specialty courses in the Leadership and Management program will have an authentic performance assessment that scaffolds the tasks of the healthcare improvement project (HIP) through the project management phases of project initiation, planning, implementation, and evaluation. Each performance assessment will focus on aspects of the project management phases by requiring a real-world approach to improving healthcare with the implementation of a healthcare improvement project. You will describe each phase of the project management life cycle in the corresponding sections of the attached “D158 HIP Paper Template.”
In this performance assessment, you will strategically plan the execution of your healthcare improvement project (HIP) to help develop the skills for systems thinking, problem-solving, and data-driven decision-making. Planning the implementation of your HIP involves identifying people, processes, and procedures that need to be in place. In addition, sociodemographic data on the population that may be affected by the healthcare improvement project need to be analyzed to determine risks and opportunities. It is also important for you to perform an assessment of the forces for and against implementing the project. Finally, it is important for you to identify short-term objectives and create action plans that align with the vision, mission, and values of the organization where the project will be implemented.
This task requires the submission of the provided “D158 HIP Paper Template,” including the following sections of your HIP paper, which you will be developing in this performance assessment:

The following templates should be completed and submitted as appendices to your provided HIP paper template:
•   SMART Goal Worksheet (completed in D156) (Appendix A)
•   Gantt Chart (completed in D156) (Appendix B)
•   Action Plan Template (Appendix C)
•   Force Field Analysis Template (Appendix D)
During your clinical practice experience (CPE), you had the opportunity to submit the force field analysis and Gantt chart. These deliverables were evaluated in your CPE for completion, not quality. When you submit these deliverables as appendices to your HIP paper in this task, they will be evaluated on content and quality of completion. The CPE provides an opportunity for you to practice and improve these items before including them in your performance assessment.
REQUIREMENTS
Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. The similarity report that is provided when you submit your task can be used as a guide.
You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.
Tasks may  not be submitted as cloud links, such as links to Google Docs, Google Slides, OneDrive, etc., unless specified in the task requirements. All other submissions must be file types that are uploaded and submitted as attachments (e.g., .docx, .pdf, .ppt).
Implementation Approach
A. Evaluate your proposed HIP implementation approach by doing the following:
1. Describe how you would organize and lead project meetings, including the method you plan to use to reach consensus on meeting agendas. Include a citation to support how consensus will be reached.
2. Explain the process you used to determine whether the organization has existing policies that could affect the proposed implementation of your project. Report the results of your review. If a policy change is required, explain your organizational process to change the policy.

B. Submit the action plan that you developed during the CPE as Appendix C of your “D158 HIP Paper Template.” Include the following elements:
•   the action item owner’s role or title
•    five action items (specific, high-level)
•   completed phase action item (e.g., initiation, planning, implementation, control phase of implementation, and evaluation/closure)
•   the anticipated end date aligned with the SMART goal end date 
Gantt Chart
C. Update your Gantt chart from D156 by doing the following:
1. Integrate the action items from your action plan into the Gantt chart by including specific tasks needed to complete  each action item.
2. Explain how you gathered information for  each component of your action plan and Gantt chart using the following key steps:
•   Define specific tasks.
•   Sequence the tasks.
•   Estimate the duration of  each task.
•   Develop the schedule.
3. Submit a clearly visible screenshot of your original Gantt chart from D156 and a screenshot of your updated Gantt chart as Appendix B of your “D158 HIP Paper Template.” 
Social Determinants of Health and Target Population
D. Describe the organization’s patient population by doing the following:
1. Describe  five general sociodemographic characteristics of your organization’s target population.
2. Describe the  five areas of social determinants of health (SDOH) of the organization’s target population.
3. Explain  both how your proposed HIP would affect your organization’s client population and how it would impact  one of the target population’s SDOH.
Force Field Analysis
E. Evaluate the existing forces for and against the proposed implementation of your project by doing the following:
1. Submit the “Force Field Analysis Template” that you completed in your CPE as Appendix D of your “D158 HIP Paper Template,” and include  each of the following components:
•    three existing driving forces that promote the proposed implementation of your project
•    three existing restraining forces that restrain the proposed implementation of your project
2. Explain how you would harness  all  three of the driving forces to implement your project.
3. Explain how you would mitigate the effects of  one of the restraining forces to implement your project. 
Scope Statement
F. Write a scope statement that includes each of the following components (from D156, D157, and D158):
•   project justification
•   the SMART goal
•    two process key performance indicators (KPIs)
•    two deliverables aligned with the process KPIs 
Communication and Training Plans
G. Create a communication management plan for your proposed project implementation by including detailed information on the following components:
•   intended receivers
•   context
•   communication outcomes
•   key messages
•   message medium
Note: You may submit your communication plan in a table or narrative form.
H. Create a training plan for your proposed project implementation by including detailed information on the following components:
•   key topics
•   training method
•   training schedule
•   training outcomes (e.g., evaluations, test scores)
